As part of the onboarding process, please complete the following activities.
Download the following documents by clicking on the images (Form W-9, Direct Deposit Form, and Handbook Acknowledgement Form).
Download the Handbook by clicking on the image.
Complete the W9, Direct Deposit Form, and the Handbook Acknowledgement Form. Change the filename to your First Name and Last Name in the place of FIRSTNAME and LASTNAME.
Take a picture of a voided check associated with the account listed on the Direct Deposit form. Change the name of the image to include your First Name and Last Name.
Once you have filled in the W9, Deposit Form, the Handbook Acknowledgement Form,and have available a picture of a voided check, upload these four files:

- Dir Deposit Form
- picture of voided check
- Handbook Acknowledgement
